- By the grant agreement No. TF0A8367 dated September 27, 2018, the Japanese Social Development Fund (JSDF) through the International Bank for Reconstruction and Development (hereinafter - the World Bank) provided the Government of Ukraine with a grant for the implementation of the Project " Community-based Social Service Delivery " (hereinafter - Project). Part of the funds of this grant may be used to cover costs within the framework of the contract to which this REQUEST FOR QUOTATIONS (hereinafter - the Request/RFQ) relates.
- Ukrainian Social Investment Fund (hereinafter - the Buyer/USIF) by this letter invites eligible bidders to send price offers for the supply of the goods, described in Annex 1 " Delivery Conditions ".
The selection of the Supplier will be carried out in accordance with the World Bank Procurement Rules for IPF borrowers from July 2016, revised November 2017 and August 2018.
Detailed information on the quantity and technical requirements for the goods are specified in the Annexes to this Request.
Note: All references to a specific brand, manufacturer, firm, patent, design or type of procurement item, source of origin or manufacturer should be read and interpreted as "or equivalent".
Equivalent (analogue) of the product means - a product that is equivalent, equivalent or better (all characteristics of the product must match or be better) in relation to the characteristics of the product indicated in this Invitation. Equivalents may be used interchangeably to achieve the same or better result.
- Bids must be complete (include all items in Table 1 of Annex 1 "Delivery Conditions") in accordance with this Request except for requirements, which are not mandatory. Incomplete proposals will be rejected.
Bids will be evaluated on all items and the contract will be awarded to the firm that offers the lowest estimated cost of all items.
- Quotations in the Ukrainian language in the form given in Annex 3 "Price Quotation ", as well as the necessary additional information, should be sent to the following e-mail and postal addresses:

Interested organizations have the right to submit price quotations both for each lot separately and for all lots at the same time.
The price offer must be submitted with the following title:

- Participants must meet the qualification requirements, which include:
- for Lot 1: successful execution of at least 2 (two) similar contracts (for the supply of furniture) in the amount of at least UAH 565,000.00 each, or 1 (one) similar contract for the amount of at least UAH 1,130.000.00 starting from January 1, 2019;
- for Lot 2: successful execution of at least 2 (two) similar contracts (for the supply of furniture) in the amount of at least UAH 390,000.00. each, or 1 (one) similar contract for the amount of at least UAH 780,000.00, starting from January 1, 2019
In the case when Quotations are submitted for two lots, the principle of compliance with the criterion regarding the execution of similar contracts for the corresponding amount must be followed for each lot for which the Quotation is submitted.
- The deadline for submitting of Quotations the e-mail specified in point 4 above: 20/03/2023, 5:00 p.m. local time.
- You must attach to your proposals the relevant documentation, catalog(s) and other printed materials or other necessary information for each item of goods (e-mail is allowed).
- (i) PRICES. Prices must be expressed in Ukrainian Hryvnia or US Dollars, excluding VAT, including customs duties, the cost of delivery, loading and unloading services, and installation of goods at the destination, which is specified in Annex 1 "Conditions of Delivery"
The delivery of the Goods will be carried out within the framework of an officially registered project of international technical assistance, which, in accordance with the Resolution of the Cabinet Ministers of Ukraine dated 15.02.2002 No. 153 and Part 2 Clause 197.11 of Article 197 of the Tax Code of Ukraine, is the basis for the supplier to receive tax benefits, in particular, exemption from VAT calculation.
(ii) EVALUATION OF QUOTATIONS. Quotations deemed to satisfy the technical requirements of the Request will be evaluated by comparing the total price. For the purpose of comparison, the Buyer will convert all prices into the currency of the Buyer's country (Ukrainian hryvnia) at the official exchange rate of the hryvnia for foreign currencies published by the National Bank of Ukraine on the date of the deadline for submitting of Quotations established in clause 6 of this Invitation.
The Buyer will determine the estimated value for each Quotation by adjusting the price offer to correct arithmetic errors as follows:
a) if in any place there is a discrepancy between the amount in numbers and in writing, the amount in writing will be considered correct;
b) if at any point there is a discrepancy between the unit price and the total amount calculated by multiplying the unit price by the quantity, the unit price shall be deemed to be correct;
c) if the Supplier refuses to accept the specified adjustments, his Quotation will be rejected.
(iii) AWARD OF THE CONTRACT. The contract will be awarded to the firm that offers the lowest overall price and meets the specified delivery conditions, technical and qualification requirements. A contract will be concluded with the selected Supplier.
(iv) VALIDITY PERIOD OF QUOTATIONS: The proposed price quotations must be valid for forty-five (45) days from the date of the deadline for submitting of Quotations established in Clause 6 of this Request.
- INSPECTIONS AND AUDITS
9.1 The Contractor shall comply with all instructions of the Buyer that are consistent with the applicable laws of the destination.
9.2 The Contractor shall permit and shall cause its subcontractors and subconsultants to permit, the World Bank and/or persons appointed by the World Bank to inspect the accounts and records relating to the procurement process, selection and/or contract execution, and to have such accounts and records audited by auditors appointed by the World Bank if requested by the World Bank. The Contractor’s and its Subcontractors’ and subconsultants’ attention is drawn to Annex IV (Fraud and Corruption) of World Bank Procurement Rules for IPF borrowers which provide, inter alia, that acts intended to materially impede the exercise of the Bank’s inspection and audit rights constitute a prohibited practice subject to contract termination (as well as to a determination of ineligibility pursuant to the Bank’s prevailing sanctions procedures).

2. Fixed price
The above prices are fixed, include all customs duties, the cost of delivery services, loading, unloading to destinations and installation of furniture at destinations and are not subject to any changes, including the period of the Contract execution.
3. The right of the Buyer to change the quantity of goods during the award of the Contract
The Buyer reserves the right at the time of awarding the Contract to increase or decrease by 1-15% the number of goods and related services specified in the RFP, provided that no changes are made to unit prices and other delivery conditions.
4. Delivery terms and conditions
The goods must be delivered within 60 (sixty) calendar days from the Contract signing date.

The Supplier is required to provide the following related services: loading and unloading of goods, installation, personnel training (if necessary).
- Payment
One hundred percent (100%) of the total price of the delivered Goods will be paid by the Buyer to the Supplier within thirty (30) working days from the day the Supplier provides the original payable invoice, sales invoice signed by the Buyer, the Act of acceptance and handover of the goods by the Recipient, and after the Supplier has performed for each delivery all obligations under the Agreement, except for warranty obligations.
If the currency of the price offer differs from the Ukrainian hryvnia - residents of Ukraine will be paid in Ukrainian hryvnia at the official exchange rate of the National Bank of Ukraine on the day the Buyer signs the invoice/document confirming receipt of the goods by the Buyer. With non-residents of Ukraine, the settlement will be made in the Contract currency.
6. Warranty obligations
Delivered goods must be guaranteed by the Supplier for at least twelve (12) months from the date of delivery of the goods. The Supplier provides the Buyer with warranty documents for the goods together with the payable invoice and the sales invoice.
During the warranty period, all defects must be corrected by the Supplier at its own expense no later than 15 days from the date of receipt of the notification from the Buyer.
7. Consequences of non-fulfillment of the ontract by the Supplier
The Buyer has the right to terminate the Contract without any obligations to the Supplier in case of failure to deliver the Goods in accordance with the above conditions within 21 days after the relevant written Buyer’s notification.
8. Technical requirements
Are listed in Annex 2 to the RFP. The supplier must confirm the compliance of the proposed goods with the specifications for each item or cite all discompliance.
9. Packaging and labeling instructions
The Supplier shall carry out the standard packaging of the Goods, which is required to prevent their damage or deterioration during transportation to the destination.
10. Defects and shortcomings
All defects and shortcomings must be corrected by the Supplier at its own expense within 15 days from the date of notification by the Buyer.

Annex 2. Technical requirements
Lot 1 "Rehabilitation equipment (electromechanical)"
1. General requirements for the subject of procurement:
1. The goods offered by the Bidder shall meet the medical and technical requirements set forth in this Annex.
The compliance of the technical characteristics of the procurement item offered by the Bidder with the medical and technical requirements must be confirmed by reference to the relevant sections and/or pages of the manufacturer's technical documents (manufacturer's prospectus and/or operating instructions and/or manual and/or technical description or technical specifications and/or other manufacturer's technical documents in Ukrainian and/or Russian), which contain this information, together with the original or certified copy of the relevant documents. Confirmation of medical and technical requirements is provided in the form of a completed table.
2. The goods offered by the Bidder shall be put into circulation in accordance with the legislation in the field of technical regulation and conformity assessment, in the manner prescribed by law.
The Tenderer shall provide:
a) a copy of the documents confirming the possibility of putting into circulation and/or operation (use) of the medical device based on the results of the conformity assessment procedure in accordance with the requirements of the technical regulations at the time of submission of proposals
(Certificate of Conformity and/or Declaration of Conformity);or
b) a letter of guarantee to provide copies of documents confirming the possibility of putting into circulation and/or operation (use) of the medical device based on the results of the conformity assessment procedure in accordance with the requirements of the technical regulations at the time of delivery of the proposed product.
3. The goods offered by the Bidder must be new, not earlier than 2022, and not used.
As a confirmation, the Bidder shall provide a letter of guarantee.
4. The warranty period (term) of operation of the offered Goods shall be at least 12 months.
As a confirmation, the Bidder shall provide a letter of guarantee.
5. The Bidder shall provide qualified training to the Customer's personnel on the use of the proposed equipment.
As a confirmation, the Bidder shall provide a letter of guarantee on the provision of training of the Customer's personnel in the use (management) of the equipment at the place of its operation.
6. The Bidder shall confirm the possibility of supplying the Goods offered by it in the quantity and within the terms specified in this Documentation and the Bidder's offer.
As a confirmation, the Bidder shall provide an original letter from the manufacturer or its official representative in Ukraine, or a dealer, distributor officially authorized by the manufacturer, confirming the possibility of the Bidder to supply the goods being the subject of this bidding in the quantity and within the terms specified in this Documentation and the Bidder's offer. Lot 2 "Rehabilitation equipment (mechanical)"
1. General requirements for the subject of procurement:
1. The goods offered by the Bidder shall meet the medical and technical requirements set forth in this Annex.
The compliance of the technical characteristics of the procurement item offered by the Bidder with the medical and technical requirements must be confirmed by reference to the relevant sections and/or pages of the manufacturer's technical documents (manufacturer's prospectus and/or operating instructions and/or manual and/or technical description or technical specifications and/or other manufacturer's technical documents in Ukrainian and/or Russian), which contain this information, together with the original or certified copy of the relevant documents. Confirmation of medical and technical requirements is provided in the form of a completed table.
2. The goods offered by the Bidder shall be put into circulation in accordance with the legislation in the field of technical regulation and conformity assessment, in the manner prescribed by law.
The Tenderer shall provide:
a) a copy of the documents confirming the possibility of putting into circulation and/or operation (use) of the medical device based on the results of the conformity assessment procedure in accordance with the requirements of the technical regulations at the time of submission of proposals
(Certificate of Conformity and/or Declaration of Conformity);or
b) a letter of guarantee to provide copies of documents confirming the possibility of putting into circulation and/or operation (use) of the medical device based on the results of the conformity assessment procedure in accordance with the requirements of the technical regulations at the time of delivery of the proposed product.
3. The goods offered by the Bidder must be new, not earlier than 2022, and not used.
As a confirmation, the Bidder shall provide a letter of guarantee.
4. The warranty period (term) of operation of the offered Goods shall be at least 12 months.
As a confirmation, the Bidder shall provide a letter of guarantee.
5. The Bidder shall provide qualified training to the Customer's personnel on the use of the proposed equipment.
As a confirmation, the Bidder shall provide a letter of guarantee on the provision of training of the Customer's personnel in the use (management) of the equipment at the place of its operation.
6. The Bidder shall confirm the possibility of supplying the Goods offered by it in the quantity and within the terms specified in this Documentation and the Bidder's offer.
As a confirmation, the Bidder shall provide an original letter from the manufacturer or its official representative in Ukraine, or a dealer, distributor officially authorized by the manufacturer, confirming the possibility of the Bidder to supply the goods being the subject of this bidding in the quantity and within the terms specified in this Documentation and the Bidder's offer.
